Accessing Your Plan Documents

You can easily view and download your plan documents from the CalChoice website by following the steps below:

Step 1

Go to www.calchoice.com.

Step 2

Click on Forms & Documents at the top of the homepage, then select the Documents tab.

Step 3

Use the search filters to find your plan documents:

  • Effective Date: Select December 2025
  • Document Type: Choose either:
    - EOC (Evidence of Coverage), or
    - SBC (Summary of Benefits and Coverage)
  • Carrier: Select your insurance carrier
  • Benefit Level: Select your plan level
  • Language: Choose English or Spanish

Step 4

Click the Search icon.

Step 5

From there, you can view the document or email a copy to yourself.

Tip: If you’re unsure which plan you enrolled in, you can confirm your plan details by logging into your benefits platform, Ease.
